Health Information
Some information Quannet Health Inc. collects constitutes protected health information (“PHI”) under the U.S. Health Insurance Portability and Accountability Act (“HIPAA”). As set forth above, Quannet Health Inc. (or your own medical provider if you do not use a Quannet Health Inc. Medical Provider) will provide you with a Notice of Privacy Practices describing its collection, use and disclosure of your PHI, not Quannet Health Inc.. Quannet Health Inc. is a “business associate” (as that term is used under HIPAA) that provides services to and for Quannet Health Inc. Quannet Health Inc. will use and disclose PHI only in accordance with the business associate agreements and HIPAA.
